Thank you for your interest in being a patient at Insight Choices. We are accepting new patients for medication management (Psychiatry) and we currently have a waitlist for talk therapy (Psychotherapy) on a first come first serve basis. Please book today for an immediate Telehealth appointment.

NEW PATIENT PROCESS

Fill out the appointment request form online or call our office at 323-375-0950. Please either scan or take a picture of the front and back of your insurance card if you choose to use insurance and a California ID. You will be asked to upload the images during the registration process. Accepted file formats are jpg, jpeg, png or pdf. Once your account is set up, you are required to complete our Clinical Intake Form and Service Agreement before we can schedule your first appointment.

USC, UC and Loyola Marymount students please note that you need to obtain a referral from your health center before completing the registration process.

Usually within 24 hours after submitting the form, a patient care coordinator will contact you to review and verify your information. The patient coordinator will schedule an initial assessment appointment. You will be provided a behavioral health questionnaire which we ask you to fill out prior to your initial assessment appointment and appointment(s) will be scheduled.

Our self-pay discounted Fee Schedule is as follows:

  • Prescriber New Patient Visit: $250
  • Prescriber Follow-up Visit: $180
  • Prescriber Extended Follow-up Visit: $250
  • Therapist New Patient: $160
  • Therapist Follow Up: $120
  • TMS: $250

CANCELLATION POLICY

As a courtesy, our scheduling system will send you an email or text reminder for your upcoming appointment. Notice that missed appointments are not covered by insurance. If you need to cancel or reschedule your appointment please do so 24 hours prior to the appointment, to avoid being charged a missed appointment fee. 

All patients who have not been seen in our practice for over one year will be subject to an initial evaluation. We ask our patients to schedule regular follow up sessions immediately after the visit to maintain treatment regularity. We collaborate as a group practice of like-minded professionals. Our providers do not always prescribe medication. The provider will see a patient for an initial evaluation, after which he or she will recommend medication treatment only if they believe it is appropriate. If the provider believes that no medication is needed, he or she will refer the patient to a psychotherapist who will provide appropriate care without the use of prescription medicine.
